Friday 8am - 7pm ET Call 734-361-5206 or fill out our Request Form and someone will get back to you. ## How It Works If you are a Blue Cross*®* Blue Shield*®* of Michigan (BCBSM) member who has a urological condition: 1 2 3 4 Please call us at 734-361-5206 or complete the Request Form. A Care Navigator Team member will contact you to discuss your symptoms over the phone. We develop a custom care plan and work with you to quickly get an appointment with the right specialty provider. Your dedicated Care Team will provide ongoing support to check-in on your progress and more support to help you feel better faster. We help you get on a path to recovery. ## Frequently Asked Questions ### Who qualifies for this service? This service is currently available to a select group of BCBSM Members. Contact your insurance carrier to find out if you qualify for this service. ### What conditions can the Care Navigation Team support? Care Navigators at TRIARQ Health assist eligible BCBSM members with recurring, persistent, or ongoing urological conditions,  providing personalized guidance and support. Conditions include urinary tract infections (UTI) and kidney stones. ### Is there a cost to this service? If you are an eligible BCBSM member, using the TRIARQ Care Navigation service is free. Your visits with medical providers are subject to your co-pay and deductible as mandated by your BCBSM coverage. ### Why should I sign up for Care Navigation Navigating healthcare can be overwhelming.
