A Care Navigation Team member will contact you within 2 business hours to confirm your eligibility and discuss your symptoms over the phone.

We develop a custom care plan and work with you to quickly get an appointment with the right specialty provider.

Your dedicated Care Team will provide ongoing support, including check-ins, appointment setting, progress tracking, and more to help you feel better faster.

Who qualifies for this service?
This service is currently available to a select group of BCBSM Members. Contact your insurance carrier to find out if you qualify for this service.

What conditions can the Care Navigation Team support?
Care Navigators at TRIARQ Health assist eligible BCBSM members with recurring, persistent, or ongoing urological conditions,  providing personalized guidance and support. Conditions include urinary tract infections (UTI) and kidney stones.

Is there a cost to this service?

If you are an eligible BCBSM member, using the TRIARQ Health Navigator service is free. Your visits with medical providers are subject to your co-pay and deductible as mandated by your BCBSM coverage.
