We develop a custom care plan and work with you to quickly get an appointment with the right specialty provider.

Your dedicated Care Team will provide ongoing support to check-in on your progress and more support to help you feel better faster.

Who qualifies for this service?
This service is currently available to a select group of BCBSM Members. Contact your insurance carrier to find out if you qualify for this service. 

What conditions can the Care Coordination Team support?
Care Coordinators at TRIARQ Health assist eligible BCBSM members with recurring, persistent, or ongoing joint and muscle pain, providing personalized guidance and support. Conditions include osteoarthritis, chronic lower back pain, and hip/knee pain.

Is there a cost to this service?
If you are an eligible BCBSM member, using the TRIARQ Care Coordination service is free. Your visits with medical providers are subject to your co-pay and deductible as mandated by your BCBSM coverage.
